I came across this Dial 'H' for Hero form, and George has confirmed with
me that he drew the two outline characters featured on the form. Scan from goodcomics.comicbookresources.com

One last comment, a non-Green Lantern one, from indefatigable Nelson Jimenez of Stamford, Conn.:
I have to disagree with the fan's choice
of "Dial H for Hero" as one of the worst series (John Genduso in CBG
#1505). This title was part of the long history of DC's Adventure
Comics and it featured the artwork of Carmine Infantino and covers by
George Pérez. Even the great writer Harlan Ellison created a character
for the series. And my major reason for not panning this title is that
I created three characters (Doomster, Chondak, and Ragnarok the Cosmic
Viking), for the series and they were all featured on various covers!
When is the new H.E.R.O. series coming out? I just hope it's not a Vertigo title!
